Contributing factors to a person's changed behaviour are often specific to the individual and may relate to something in the past or an attempt to communicate an unmet need.

In order to understand why a behaviour may be occurring it is necessary for staff to learn about the person and why they are responding in this way.

The Lifestyle and social history questionnaire captures information and improves understanding of the person and enables care tailored to the person’s needs.
