DSA regional "clinical-style" model

DSA has always been committed to ensuring equal access to dementia care across the nation, going anywhere at any time to meet the needs of people living with dementiaDSA is now on a mission to further build upon meeting those needs, and the increasing challenges, through the regional “clinic-style” model.  

To be a contributor in breaking down barriers in rural and remote areas, DSA engages in discussions with consultants, reviews cases and referrals, and works with requests from the Aged Care Quality and Safety Commission and care homes, enabling us to understand the unique challenges of each region.  To date, these have included changes in routine care, staff turnover, environmental factors, and communication breakdowns. With this increased understanding, DSA is working in collaboration with care homes, families, and communities to address frequent or complex referrals where it is evident there is a need for capacity building.    

Dementia Support Coaches work with care home staff to implement immediate strategies in support of possible contributing factors and influence Behaviour Support Plans.  Together with the Dementia Consultants they refer, when appropriate, to specialised teams for additional support, including environmental audits to highlight the link between how the environment can impacts on a person.  Educational resources and training are provided during regular visits by the Dementia Consultants who conduct toolbox sessions to address common themes.  

One of the key ingredients of the clinic's success involves the exploration of, and connecting with relevant community organisations to bridge gaps, share success stories, and increase opportunities for motivation in the presence of stable environments.
