This activity planner is designed to help carers and health professionals build everyday connections with a person living with dementia.

Rather than focusing on outcomes, this tool encourages meaningful moments through presence, patience and understanding. It provides a simple way to plan activities based on the person’s mood, energy levels and what matters to them, supporting comfort, engagement and dignity throughout the day.

Use it to reflect on what works, adapt your approach, and support more connected, person-centred care at home or in a care setting
