Behavioural Assessment Form

When a changed behaviour is identified it is important to assess the changed behaviour and complete a Behaviour Assessment Form.

The Behaviour Assessment Form (BAF) provides a comprehensive record of the changed behaviour and includes an analysis of the things that happened before the behaviour, the behaviour itself and any consequences.

The BAF is a tool to help better understand a behaviour and to inform the person’s Care Plan and Behaviour Support Plan. The BAF should be completed whenever a significant incident occurs including when:

  • a new type or change in the changed behaviour is observed
  • a change in the pattern of a behaviour needing support, including when it happens more often or with greater severity
  • a change in the person’s response to previously effective strategies to the behaviour needing support
  • when a series of assessment forms are being completed in order to improve our overall assessment of a person and their behaviour.
