Behaviour Frequency Chart

Identifying a changed behaviour and communicating to the care team about what is happening is essential to the wellbeing of the resident. The Behaviour Frequency Chart allows documentation of the changed behaviour in a full and accurate record.

The Behaviour Frequency Chart (BFC) is a simple tool to record the number of times a specific behaviour is happening and helps to identify when a behaviour occurs during the day and when or whether different interventions are effective in reducing the behaviour.

Depending on the behaviour, the BFC may be used multiple times a day. The BFC should be used until effective strategies are in place to reduce the frequency of the behaviour.
